Terms & Conditions – $69 Family & Friends Referral Promotion

Eligibility

• This promotion is available exclusively to current and past patients of Johns Creek Chiropractic and Wellness Center and the family members, friends, and personal referrals they invite.

• The offer may not be transferred or shared outside of this network.

Offer Details

• The promotional fee is $69 for a Complete Chiropractic Evaluation (normally $129).

• The evaluation includes consultation, examination, and scans, one set of  X-rays, if clinically necessary, one chiropractic adjustment, and a hydrotherapy session. All services must be completed during the first visit.

• This promotional fee is a discounted, introductory rate and cannot be combined with insurance, other offers, or discounts.

Completion Requirement

• To qualify for the $69 price, the referred patient must schedule AND complete their first visit on or before September 30, 2025.

• Scheduling an appointment alone does not guarantee the promotional rate if the appointment is missed, rescheduled beyond the deadline, or not completed.

Limited Availability

• The promotion is limited to the first 50 completed visits. Once 50 patients have scheduled and completed their first visit, the promotion will automatically end, even if the expiration date has not been reached.

Expiration

• This offer expires on September 30, 2025, or once 50 completed visits are reached—whichever comes first.

Non-Transferable & No Cash Value

• This promotion has no cash value and cannot be redeemed for cash, credit, or future services.

• The $69 fee applies to one evaluation per referred patient.

Office Rights

• Johns Creek Chiropractic reserves the right to modify, cancel, or discontinue this promotion at any time if circumstances require.

• Clinical recommendations following the evaluation are separate from this promotion and will be discussed individually with each patient.
